Please meet with your supervisor to complete this form together during the first week of your internship. A completed Work Plan is required of your participation in the UCDC program. Upon submitting the Work Plan, a copy will be emailed to both you and your supervisor.
If you have more than one internship, you will need to complete a separate Work Plan for each internship. You will be given the opportunity to return to the start of the form upon submission.
The Work Plan will provide you with guiding questions to help you set intentional goals and understand expectations for your internship this term. If you have already had a conversation with your supervisor, you may fill out this form on your own.
This form will also collect information on the schedule, modality, and compensation for your internship. Information on compensation will be used to determine your eligibility for the Subsidizing Unpaid Interns Program (SUIP). [Not applicable for summer terms]
